You do not need to fill this form.

As you have not been in contact with someone who has COVID-19 OR have symptoms of COVID-19 we recommend that you undertake a PCR test to confirm if you have COVID-19. If you receive a negative test result from your PCR, you will be free to leave isolation immediately, you will not require clearance from the Department of Health.
